We understand that surrendering your beloved pet can be a difficult decision. When circumstances arise where you can no longer care for your cat, surrendering them to a rescue is often the best choice to ensure they find a loving forever home.

I hereby forever release, discharge, and agree to hold harmless and indemnify Midwest Siamese Cat Rescue, its board of directors, its members, officers and agents from all claims, demands, actions, causes of action or liability of any kind whatsoever that are related in any way, directly or indirectly, to The Cat, including but not limited to, all claims related to The Cat’s adoption or disposition.
